NOTE: When registering & paying for a sponsorship via our site, Sports Connect (our vendor) will ask for you to create a "player" profile with your name, gender and data of birth. Name is the only important info so you can put anything in the other fields (e.g. DOB = 1/1/2000) and it will be fine. 
The form will also ask for your business name (as you would like it to appear on our site/banners), mailing address, business website and logo image or URL.
